John Scott, MD, is board certified in internal medicine and geriatrics. He will facilitate small groups of Shared Appointments for Seniors (SAS) and their family members or caregivers, and provide in-home private evaluations for individuals or couples.

SAS are designed to be similar to shared medical appointments (SMAs). SMAs are an innovative, interactive method of health care delivery that:

  • Improve patient satisfaction.
  • Prevent disease complications and hospitalizations.
  • Offer extended educational time with a doctor or other medical provider.
  • Give participants a unique, dynamic, fun, and compassionate learning experience.

Dr. Scott is the creator of one model of SMAs, and has used this model for nearly 20 years to care for seniors with complicated medical problems.

During meetings with Dr. Scott, he will review the current medical care plans, but not assume or deliver medical care, although he will communicate with participants’ personal physicians. SAS provide interactive health care counseling to help seniors:

  • Maintain independence.
  • Prevent falls (hip fractures).
  • Minimize prescriptions and drug interactions.
  • Decrease hospitalizations and emergency room use.
  • Understand health and disease prognosis.
  • Maximize quality of life.

Family members and caregivers will learn how best to provide support, and plan for the future. Discussions will encourage informed end-of-life planning (advanced directives). Waiting until a loved one is ill often leads to difficult, stressful, and uncertain decision-making. Early, and facilitated, discussions about the end-of-life allow for death with dignity, and clarity within the family when the time comes.

Seniors will also benefit from Nutrition and Lifestyle Counseling.
